Thank you for participating in the FEAST program.
On the following pages, there are some questions we would like to ask you. If you do not understand a question, please ask for help from the classroom teacher.
This is NOT a test, there is no right or wrong answer - just answer the questions as is right for you.
You will be asked to complete this survey twice - once now and once more at the end of the program. Taking part is up to you and you do not have to agree to participate.
Your individual information will be kept private and confidential.
How privacy and confidentiality will be protected:
No school, class, teacher, student or individual name will be included in the reporting of the results. It will not be possible to identify students taking part in this evaluation. The data is stored securely for a period of at least five years. At the end of five years, the material will be destroyed. Any information obtained in connection with this project that can identify students will remain confidential in compliance with the Privacy and Personal Information Protection Act 1998.
